Introduction: Obesity is a momentous national health concern (CDC, 2020; Cawley et al., 2021). Healthcare providers are required to assist and support their patients to address obesity. However, research is needed to determine how healthcare provider weight may affect patient trust in receiving and acting on weight loss advice. The purpose of this study was to determine patient willingness to discuss weight loss with their primary care provider (PCP), and patient opinions about diet change, exercise, or weight loss advice from their providers. A related purpose was to determine if PCPs were discussing their personal health practices with their patients (known to promote patient health) and PCP response to a review of the results of the patient sample data and its expected impact on their practice. Methods: the study was a descriptive survey design and used Qualtrics to distribute a patient/community member instrument with questions related to patient trust in and receiving weight loss advice from PCPs. A related survey of PCPs in a selected health system determined PCP practices in working with overweight and obese (OW/O) patients. In addition, PCPs were asked to review the community member survey results and comment on possible impact on their practice with overweight and obese patients. Results: Using X2 analyses, the patient participants were significantly more likely to recommend to others a normal weight provider as opposed to an OW/O provider. They believed that a normal weight provider could more effectively plan a weight loss program and would better understand the difficulty in losing weight. Finally, patient participants were significantly more likely to be emabarrassed in discussing weight loss with an OW/O provider. Physician participants who reviewed patient data were more likely to increase their counseling regarding weight loss topics. Conclusions: The current study identified significant patient concerns about accepting diet, exercise, and weight loss advice from an OW/O provider compared to a normal weight provider. Physician participants who reviewed patient data were more likely to increase their counseling regarding weight loss topics. This study identifies the significant problem of OW/O providers in their role to provide weight loss advice. Patients are embarrassed to ask them about weight loss and obese providers do not often raise the topic. There are no easy answers to this dilemma. Realistically, it may be helpful in the short term to develop and investigate new approaches for providers with a weight problem to interact more effectively with their patients regarding weight loss.

Background: It is widely accepted that harassment in the workplace may have negative effects on the mental and physical health of nurses. We hypothesized therefore that short schema psychotherapy may reduce chronic anxiety symptoms thus improve nurses quality of life. Purpose: This study is sought to identify the effectiveness of anonymous group psychotherapy via teleconference on victims of workplace harassment in nursing personnel working on NHS of Greece. Method: A case-control study enrolled 14 nurses victims of workplace harassment were randomly allocated in parallel and anonymous psychotherapy groups during August 2017 and January 2018. A qualified psychologist delivered a schema method to reduce chronic anxiety in the intervention group. Validated screening tools were used to assess anxiety State-Trait Anxiety Inventory (STAI) and the Hamilton Hamilton Anxiety Rating Scale (HAM-A) to assess the intensity of the anxiety. Individuals characteristics were also assessed. P-values >0.50 were considered statistically significant. Results: Significant chronic anxiety reduction were presented in the psychotherapy group compared to the control group (p-value = 0.038). In particular, individuals before the intervention (week 0, t = 0), presented with a higher level of chronic anxiety (STAI= mean 81.42) in comparison to (mean 67.28 - week 14, t = 1) after the intervention, suggesting that the psychotherapy schema was efficient. Similarly, decreased anxiety symptoms were also found after the intervention (mean 9.71 to 4.85) using the Hamilton Anxiety Scale. No other significant differences were found even though most of the participants were females. Conclusions: Our data suggest that anonymous group psychotherapy via teleconference is a safe and effective method to reduce chronic anxiety in victims of workplace harassment.

Background:Antibiotics are overused. Resistance rates continue to increase, unnecessary money is spent, and often unwanted side effects follow. Reducing antibiotic resistance rates is a call to action for healthcare providers (HCP), pharmaceutical companies, patients, patient family members, and agricultural breeders. The purpose of this QI project wasto use an algorithm as a clinical support tool to assist providers practicing in an urgent care setting to reduce the number of antibiotics prescribed for common acute respiratory infections (ARIs) such as pharyngitis, sinusitis, and bronchitis. Methods: The project setting was one urgent care system between two locations within the Dallas Metroplex with an annual volume of 16,000 patients. As a QI project, six providers were educated on and asked to review an algorithm tool prior to prescribing antibiotics for ARIs related complaints based on designate diagnoses (acute pharyngitis, acute sinusitis, and acute bronchitis). Using data obtained from EHR, the project compared the number of antibiotics prescribed for ARI related complaints in the intervention period compared to baseline period from same timeframe of previous year’s prescribing rates. Conclusion:A clinical decision support tool such as an algorithm along with patient and provider education can reduce antibiotic overuse burden. Unnecessary antibiotic prescribing for the treatment of acute respiratory illnesses has continued risks to patients and the community. Results:Project findings confirmed the project aim for this quality improvement project that using a clinical decision support algorithm tool leads to significant decreased antibiotic prescribing rates for the acute respiratory illnesses of acute pharyngitisX2 (1, N = 95) = 23.69, p = 0.000, acute sinusitisX2 (1, N = 94) = 9.94, p = 0.003, and acute bronchitisX2 (1, N = 95) = 23.69, p = 0.000.
